Both.
Usually, just the music (which includes the melody the vocals are singing, just not the words they say), BUT if a song has really good lyrics, that will increase my enjoyment of it. Though, bad lyrics don't usually decrease my enjoyment of a song.

Rock Band kits can easily be used with DTXMania.

I would probably just have them stand as if on flat ground for low slopes. Higher slopes, I'd use a sprite with the character's legs positioned as if they're standing on a 45 degree slope. I kind of want to play IIDX, but I never was able to...

I actually don't have too much rythym game experience, except for Guitar Hero, which I'm pretty good at (30-ish Expert Full Combos in Guitar Hero 3).
Although, I did buy Amplitude (and some other non-rythym games) yesterday, and I love it.
