Hey, same state as you so here's some tips.
The state background check can either be mailed OR you can hand deliver it to the office at Kipling & 6th Ave. I cant remember if you can expedite it or not, but they're pretty speedy.
As for the federal, the letter is a new thing as of early this year and yeah it isnt mentioned in the packet. BUT, you can go on the federal website and print out their template, its not a letter exactly. Its retarded and basically just has 5 or 6 fields, name, DOB, (etc), and purpose of search, which is legal name change. I would REALLY suggest expediting this, it took me 6 months to finally get mine back - if you do, dont forget to write expedite on the outside.

Also, I forgot to mention; they sent me back a letter saying my request had been rejected for not having a cover letter (the same as you). I called them and they said that if I faxed them something back, they could continue. I did, and had my BG check in hand two weeks later. If yours was recent, call them!
edit edit: I actually ended up getting "rejected" twice on that app, so I would strongly suggest you dont take a letter saying you did something wrong as start over. Their letters were very poorly worded.
