They usually go until the forums explode from all the confusion and frustration.

They then have to start anew, so Madnesses are few and far between.

No. Only since the creation of the FF00BF BFFs group.

Edit: Actually, I'm guessing it was recently swapped in so that Az could send out his PSA, and they subsequently forgot to remove it. But I'm sure that nobody would abuse such a thing…
